Jobing Description

Under direction and supervision of the Program Manager; plans, directs, and supervises staff in Home and Community Based Services to ensure high quality service to individuals with developmental and physical disabilities.

 Position Responsibilities

  1. Performs attendant care assessments within 30 days of new member, then quarterly thereafter. Assessments submitted to Program Manager.
  2. Schedule DSP for hours for ATC, HAH, RSP and HSK.
  3. Provides supervision and direction to program staff and ensures VALLEYLIFE policies are being followed.
  4. Updates and maintains schedules on the N:Drive.
  • Serves as technical resource to staff and staff orientation and correct completion of timesheets and main trainer for service delivered. Tracks and accurately records all DSP attendance for HCBS staff on a daily basis.
  1. Develops data and maintains appropriate information and prepares written monthly reports required by the 10th of each following month.
  2. Attends ISP, team, and emergency meetings regarding member.
  3. Provides direct care services as needed to prevent gap in service.
  4. Delivers disciplinary actions up to and including suspension.
  5. Completes performance evaluations for all staff members.
  6. On call 24/7 to meet the needs of the HCBS members and staff.
  7. Performs other related duties and assignments as assigned.

Knowledge, Skills and Abilities

  • Ability to effectively supervise staff and maintain successful working relationships
  • Ability to plan, organize and carry out program objectives.
  • Ability to effectively communicate in English both in writing and verbally.

Requirements

  • High School Diploma or equivalent is required.
  • 3 years experience working with individuals with developmentally and physically disabled.
  • 2 years supervisory experience preferred.
  • Behavioral experience a plus
