III.       PREFERRED PREREQUISITIES:          

  • Bachelor’s degree in social work or related field from an accredited university

required; Masters degree preferred

  • Two years’ experience in working with children and families and/or welfare-to-work programs
  • Knowledge of social services, welfare-to-work and community resources desirable
  • Ability to relate to people of various ethnic and socioeconomic backgrounds
  • Good organizational, follow-through skills and excellent verbal/written communication skills
  • Ability to work in a multi-cultural environment
  • Valid California “Class C” driver’s license required
  • Bilingual English/Spanish required with proficiency in English required

IV.       REPORTS TO:         Program Supervisor or Above

V.        POSITION DESCRIPTION:         

The Bilingual Case Manager partners with Social Service Agency Case Managers on referrals for individuals and families participating in the CalWORKs Family Stabilization Program who are experiencing a crisis or situation that destabilizes the family and impairs the Welfare-to-Work (WTW) client’s ability to meet WTW participation requirements.  The BCM provides crisis intervention and support, referrals (resource brokering), advocacy and case management services to families in their homes, CalWorks offices, at schools or other places in the community. In addition, this position has responsibility for coordinating with multiple service providers to prevent abuse and out of home placement care; and timely completion of all required documentation.

VI.       DUTIES AND RESPONSIBILITIES:      

  • Respond to referrals from CalWORKs workers (SSW) from the Social

Service Agency, Child and Family Services, for families identified as being at-risk

of not meeting the Welfare-to-Work (WTW) participation requirements.

  • Partner with CalWorks case managers to meet with clients at their home, children’s school, CalWorks offices, or other locations to assess client’s situation (strength based assessments)
  • Conduct a comprehensive face-to-face assessment and subsequent assessments as needed
  • Partner with CalWorks case managers to develop an individualized service plan
  • Aid families in crisis situations such as immediate need for food, clothing, employment or shelter, by providing proper referral and case management by facilitating their access to a Family Resource Center
  • Utilize a family centered, strengths-based approach to work directly with clients on issues such as (but not limited to) job readiness skills, budgeting, crisis management, health and safety for children in the home environment.
  • Attends and presents cases to the Case Management Team
  • Administer evaluation tools as required by this agency and the funder
  • Coordinate service delivery with other service providers
  • Advocates for clients as needed, i.e., problems with AFDC, Medi-Cal and other systems with which clients interact
  • Attends community meetings and/or agency/collaboration meetings/trainings as assigned.
  • Maintains files, logs, charts and other records in audit condition as required by agency or funding source

  • Maintains confidentiality in matters of agency operations, personnel, and clients
  • Maintains positive communications with all agency staff, funder, clients and visitors.
  • Supports existing agency policies, principles and mission
  • Good organizational skills and follow-through
  • Good verbal/written communication skills
  • Proficiency in Microsoft Office software programs
  • Exercise good judgment in performance of duties and responsibilities
  • Ability to relate to people of various ethnic and socioeconomic backgrounds
  • Other duties as assigned
