 Position Summary: This Case Management position is bilingual (Spanish).  The Case Manager works using discretion and independent judgement, is responsible for the delivery of quality participant care to members via the coordination of the youth’s clinical team in securing and managing general integrated health services.

 

 Essential Functions

 

  • Participates in the assessment, evaluation and planning of patient care.
  • Develops initial individualized service plans for members.
  • Provides community and home based services to enhance and achieve treatment goals.
  • Develops, facilitates and coordinates CFT/ART meetings, staffing’s, case conferences and other meetings as appropriate.
  • Provides referrals to outside providers and assists in securing a variety of social services, including, but not limited to, relapse prevention services, medical care, food stamps, general assistance and child-care.
  • Maintains a minimum number of documented client contact hours per week (30 hpw).
  • Maintains appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source requirements.
  • Communicates with member (or member’s family) to coordinate consent to care, releases and/or referrals for provider care which includes behavioral care, general medical care, community resources, educational, social, judicial and other State agencies.
  • Coordinates transportation of members to and from appointments when appropriate.
  • Consults with treatment staff to coordinate effectiveness of services contained in the members individual service plans and make adjustments where needed.
  • Provides outreach and follow-up on crisis contacts and missed appointments.
  • Meets with service providers to review utilization and monitor member progress towards program goals.
  • Provides member advocacy and education as needed, to ensure delivery of required services.
  • Develops transitional service plan (including discharge planning) with members, prior to reentry into the community.
  • Performs other related duties in accordance with agency growth and changes.
  • Preserves the human dignity and the confidentiality of each patient.
  •  All clinical staff will participate in the rotation of "on-call" duties by the carrying of the company on-call cell phone. This duty includes overnight, weekends and holidays.


Qualification Requirements

Education/Experience: 

 

  •  Bachelor’s degree in Social Work, Counseling, Psychology or a related human service field
  •  Behavioral health experience/education to qualify as a Behavioral Health Technician (BHT)
  •  Licensed Behavioral Health Professional preferred but not required
  •  Combination of education and/or experience to be effective in a position that is predominantly intellectual in character
  •  Bilingual highly preferred but not required

 

 ***MUST BE ABLE TO PROVIDE***

 

  • Valid Arizona license or valid other state license with ability to obtain Arizona license by date of hire
  • Valid automobile insurance
  • Copy of highest education certificate or transcript showing graduation
  • Valid CPR and First Aid certifications or ability to obtain within thirty (30) days of hire
  • Valid Fingerprint Clearance Card or ability to obtain within ninety (90) days of hire
  • Five (5) year MVR (driving record) that meets the agency’s auto insurance carrier’s coverage requirements

 

 Knowledge of:

 

  •  Principles and methods of psychosocial rehabilitation, family support and youth development.
  •  Basic principles of art.
  •  General business principles.
  •  The special needs and characteristics of the population being served.
  •  Gender and cultural differences and skill to be sensitive to those differences in the service delivery setting.
  •  How to prevent violent behavior or behavior harmful to the program participant or others.
  •  Methods and places to access resources for assistance.
  •  Business equipment, computers, computer software, and office procedures.

 

 Skills:

 

  •  Excellent interpersonal and communication skills.
  •  Group process and group facilitation.
  •  Show creativity, energy, and flexibility in serving as a healthy role model for staff and recipients.
  •  Use of a personal computer with good word processing and general computer skills and have ability to edit and proofread written materials
  •  Proficiency in organizing projects, prioritizing workflow and completing multiple tasks simultaneously and accurately
  •  Demonstrated ability to exercise independent judgment, initiative and tact in dealing with community stakeholders, management, supervisors, and staff.
  •  Ability to maintain confidentiality.

 

 Physical Requirements

Lifting/Pushing/Pulling:

 

  • Must be able to lift and carry boxes/packages up to 50 pounds as needed.
  • Must be able to push and pull file cabinet/desk drawers open and closed daily.

 

 Body Positions/Movements:

 

  • Sits in a chair at a desk for extended numbers of hours each workday: moves from office to office.
  • Must be able to retrieve items/supplies on overhead shelves and bend/stoop to reach files located in bottom file cabinets.

 

 Hand Use/Sense:

 

  • Manual dexterity sufficient to use calculator, telephone, keyboard and various office machines on a daily basis and be able to write.
  • Must be able to communicate both in person and on the telephone, enter information on the computer and read various instructions.
  • Be able to read numbers, reports, and computer terminals.
  • Operating an agency motor vehicle, other vehicle, equipment, machinery or power tools.
  • Must be able to analyze many variables and choose the most effective course of action. Personal maturity is an important attribute.
  • Must be able to resolve problems, handle conflict and make effective decisions under pressure.
  • Ability to give, receive, and analyze information, prepares written materials, and articulate goals and action plans.
  • Ability to do simple math calculations, input data into the computer, and analyze data.
  • Must be able to use computer and other office machines such as fax, calculator, telephone, copy machine, vehicle.
  • Works in a typical office setting, travels to various events and community locations.
