    PROGRAM DESCRIPTION:
    Traugott Terrace is an alcohol and drug free community that provides 12 units of transitional and 38 units of permanent housing to low-income adults. It is a service-enriched housing program that provides relapse prevention case management to individuals actively involved in programs of recovery. The philosophy is strongly rooted in providing housing services with dignity and building a sense of community among the residents.

    COMING HOME DESCRIPTION:

    Coming Home is a case-management program that serves formerly homeless adults for one year with the goal of housing stabilization.The Coming Home Housing Stabilization Specialist provides intensive individualized support for people who have been homeless in the City of Seattle. Services will be focused on the transition into permanent housing and the first year of tenancy. The Housing Stabilization Specialist will assist tenants in making and sustaining reliable, positive community-based support networks; in learning or relearning daily living skills required for permanent housing; and in becoming active, empowered members of their residential environment.

    Responsibilities

    MAJOR DUTIES AND RESPONSIBILITIES:

    Case Management & Relapse Prevention Services:

    • Case management oversight of residents including: initial needs assessment; creation of individualized case management agreement; individualized case management services that focus on a clean and sober lifestyle, skills enhancement, independent living, social skills and tenant responsibilities. Draw upon specialized CD knowledge and skills regarding relapse prevention planning and implementation. Establish, implement, monitor, evaluate, and revise case management plans as needed for each resident.
    • Monitor residents for alcohol and drug use; conduct random UA testing and breathalyzers.
    • Participate in screening, admission, and intake processes by consulting with Program Director and Business/Leasing Manager, utilizing specialized professional CD knowledge, skills, and experience to determine appropriate placement of applicants.
    • Collaborate with other professionals regarding resident needs and services.
    • Maintain and review accurate and current case management records and documentation in files and logs.
    • Provide current information about available resources.
    • Provide crisis intervention and conflict mediation.
    • Collaborate with local neighborhood agencies that offer needed services for Traugott Terrace residents.
    • Participate in monthly staff meetings, offering specialized CD knowledge and expertise in individual case presentation and consultation.
    • Draw on specialized CD knowledge and expertise to assist in determining what new initiatives could be developed to enhance services.

    Committee Oversight Duties:

    • Assist Program Director to screen, hire, and train Resident Night Monitors and Security Team.
    • Arrange in-service training for other staff (substance abuse, mental health, relapse prevention, etc.).
    • Promote and facilitate monthly community meetings, including preparation of the agenda.
    • In conjunction with the Program Director, oversee partnerships and contract services provided by other programs or agencies.

    COMING HOME HSS MAJOR DUTIES AND RESPONSIBILITIES:

    Intensive Transitional Services:

    • Provide intensive case management to residents for the first twelve months after move-in.
    • Verify and collect documentation of resident eligibility.
    • Interview applicants and assess for ability to live autonomously in the community, stability, and for special needs in health care, mental health, and alcohol/drug addiction. Determine which applicants require further diagnostic screening by mental health or substance abuse professionals. The interview also assesses social skills, life skills, financial needs, job readiness/needs and availability of a positive peer network.
    • In conjunction with resident, develop a specific Housing Service Plan, which will address the identified needs as well as the areas which have been a problem for the resident in maintaining previous housing.
    • Provide 1-1 service and work with new residents to develop individual growth goals.
    • Provide current information about available resources and assist tenants in making application for needed resources including income, health care, employment, documentation, and support services.
    • Provide crisis intervention – which may involve substance abuse, mental health, and medical issues.
    • Arrange access to community treatment resources and support programs by medical, mental health, and alcohol recovery providers.
    • Monitor effectiveness of response and resident satisfaction with resources and services received.
    • Act as advocate for residents on an on-going basis to ensure consistent service from agencies and systems.
    • Give input on eviction proceedings.
    • Participate in and contribute to building a safe, supportive residential community.
    • Coordinate services with other staff and outside agencies and help foster an environment of teamwork and professionalism.

    Qualifications

    MINIMUM QUALIFICATIONS: (Qualifications which applicants must possess to be considered for the position.)

    • Bachelor’s degree in a human services field; equivalent relevant experience in social services may substitute for the degree requirement.
    • Two years direct services experience, which includes providing linkage services for low-income and CD populations. Desired experience includes work with special needs populations, working with and understanding culturally diverse groups and working knowledge of community agencies and resources.
    • Case management experience in a chemical dependency setting.
    • Professional experience working with homelessness, chemical dependency, trauma, and mental illness.
    • Professional experience collaborating with licensed Chemical Dependency Professionals and Mental Health Professionals for effective case management planning & implementation.
    • Strong, independent problem-solving/issue assessment skills, intervention planning/implementation skills, and/or crisis intervention or conflict mediation skills.
    • Ability to work independently with and effectively counsel persons who are homeless and are in recovery from alcohol and/or drugs.
    • Ability to contribute to a positive work environment that fosters respect, dignity, teamwork, and supports a culturally diverse work environment.
    • Excellent oral and written communication skills.
    • Ability to work independently and as part of a team.
    • Commitment to working within the mission and values of Catholic Community Services and Catholic Housing Services.

    PREFERRED QUALIFICATIONS:

    • Experience with group facilitation.
    • Computer abilities with spreadsheets and database programs.
    • Knowledge of Landlord/Tenant laws.
