Developmental Pathways has an excellent opportunity for a Bilingual (Spanish) Intake Case Manager in our Case Management department. Our Intake Case Managers are the team who give a friendly and supportive welcome to people new to our agency - adults and children with intellectual and/or developmental disabilities/delays in Arapahoe and Douglas counties and the city of Aurora who are applying for developmental disabilities services.

You get to assist folks with completing determination and verification of disability/delay; benefit information and application support; enrollment offers; Children’s Extensive Support (CES) intakes and initial applications; and functional eligibility assessment. You’ll also provide waitlist updates and case management to individuals waiting for services, as needed; act as the systems navigator by assessing overall needs of individuals waiting for services and/or newly applying for determinations; and refer to appropriate case management agencies including Single Entry Points (SEP), community resources, and Health First Colorado (Medicaid).

This position will be based out of our Denver Metro, CO office (just south of the Denver Tech Center in Englewood, CO). Please note that our Case Management team has implemented a mobile working environment to provide the team with more flexibility in their roles. We’re committed to a great work-life balance here, so our offices are open Monday - Thursday and are closed on Fridays. This is a full-time 36-hr/week position working Mondays - Thursdays.

The Good Stuff - Starting Salary and Benefits!
$17.60 - $19.93/ hour + $1950 annual bilingual stipend paid out biweekly

  • Review and verify, or complete new disability determinations for individuals transferring to Developmental Pathways case management from other Community Centered Boards (CCBs), Regional Centers, Intermediate Care Facilities (ICFs) Jail, nursing facilities, etc.
  • Provide backup support to complete PASRR Level 2 assessments, as needed.
  • Serve as the liaison for individuals applying for or waiting for services between Case Management/Early Childhood/Community Outreach departments, families and other agencies; negotiate positive outcomes for individuals in service.
  • Support individuals to enroll into services by completing enrollment offers and related processes.
  • Support individuals to apply for Medicaid, Medicaid Disability, and Supplement Security Income (SSI) benefits as applicable, depending on the unique needs of the individual.
  • Contact individuals waiting for services at minimum annually to verify and update information, confirm needs, provide resources and/or referrals for additional supports for which they may qualify to help meet their needs.
  • Provide/coordinates needed level of support in emergency and crisis situations (ERR, waitlist SP, regional center admission/CST referral, RFP, etc.).
  • Recommend and assist individual/families to connect with relevant state-funded programs, waiver programs, community, 3rd-party, or Medicaid State plan supports as applicable.
  • Safeguard individual rights and assures that due process is followed (disputes, suspensions, informed consent, etc.).
  • Enter all relevant information into the appropriate Case Management tracking files/State databases and COSMO when needed.
  • Document contacts and activities relating to individuals in service, including enrollment/waitlist as appropriate.
  • Assure completion and submission for approval of funding-related documents.
  • Present at resource fairs, school liaison opportunities, and public awareness events as assigned to communicate information about programs and services.

Qualifications
Must have’s to be our Bilingual  Intake Case Manager!
  • Bachelor’s degree in a human services related field or 5 years equivalent education and experience.
  • Bilingual in Spanish and English with the ability to effectively read, write and speak Spanish and English, required
  • One (1) year of experience in human services or a related field.
  • Working knowledge of the developmental disabilities system, rules and regulations preferred
  • Excellent oral and written communication skills
  • Desire and dedication to provide the best customer service
  • Strong organizational skills
  • Demonstrate strong negotiation skills
  • Ability to perform computer functions necessary to carry out job responsibilities
  • Ability to work flexible hours
  • Personal vehicle in good operating condition for use during work
  • Valid Colorado motor vehicle license/proof of adequate motor vehicle insurance
  • Meet our driving requirements - no suspensions/less than three (3) tickets in the past three (3) years
