Job Profile

The Care Manager Team Lead will manage CHC members with chronic diseases, conditions or medically complicated needs through comprehensive evaluation and through coordination of resources. The Care Manager Team Lead will participate as a team member in CHC Care Management program. Care Managers will work collaboratively with utilization management, member services and the quality improvement departments.


  • RN current Texas License required; CCM preferred
  • One year hospital experience or one year managed care experience.
  • One year of case management experience
  • Preferred 1 year case management experience.


  • Above Average Verbal communication skills(Heavy Public Contact)
  • Writing /Composing (Correspondence/ Reports)
  • Analytical, Medical Terms, P.C.
